Abstract
|

|
A 74-year-old house wife was admitted to the S.N.U.H. on 28th of Jan., 1963, with an infant head sized tumor mass on the right side of nape of neck as a chief complaint. The mass was first found 10 mos. prior to admission.
Findings on admission showed a large tumor mass (12;: 6 x 6cm. surface diameter) with tenderness on the right lateral aspect of the nape. It was elastic hard, had nodular and smooth surface, and also was movable to the up and downward direction but was not to the lateral side, accompanied with extensive collateral circulation over the discolorated skin surface. Fluctuation was present.
Radiological findings of chest PA and cervical spine revealed a large tumor mass on the right side of the neck and several fibrotic calcifications on the both apex of the lung which were not compatible with metastatic lesions.
Biopsy at the OPD showed a Leiomyosarcoma originating from venous system.
At operation the tumor mass was located between the lateral side of the trachea and internal jugular vein, and was covered with the right 5terno-cleido-mastoideus muscle, which was displaced medially. It was consisted of lobulated nodular mass with extensive collateral circulation and increased vascularity, the pedicle of the mass being attached to the lateral tribuary of the internal jugular vein. It was hard in consistency with central necrosis and hemorrhage.
The histo-pathogical findings of the extirpated mass also revealed a Leiomyosarcoma, the surface of which was surrownded with hard fibrotic tissue, and a number of lomboidal shaped tumor cells and with quite few mytotic division of the cells were seen.
Postoperative course, with 5,000 r of X-ray irradiation for prevention of recurrance, was excellent up to the present.
